If you are planning to have your treatment reimbursed by medical insurance, please obtain a doctor's prescription for Physical Therapy. The prescription does not have to specify the type of treatment, only list a diagnosis, but is typically recquired by the insurance provider.
If you are not using health insurance any form of documentation of diagnosis is acceptable. A prescription for Physical Therapy works very well, as do copies of patient records, verbal communication from your doctor or dentist etc.
